WELLPATH – SCHEDULE A PICKUP

To schedule a collection, please complete the below form. 

1 to 5 Items: Fedex Labels will be sent for shipment.  Please securely pack the items and have FedEx collect the boxes, or drop off at a local Fedex Facility.

Greater than 5 Items: Based on the type, quantity and location of the assets, different collection and shipping methods may be used.  Memory for Memory will contact you once the form is received to instruct you on the collection and shipping process.
